Trick or Treat Room/trunk or treat Sign Up

This is the sign up for a room for the kids trick-or-treat event and for a car spot for the Trunk or Treat Event. The fee for the room is $15 and the fee for Trunk or Treat is $5 with all proceeds going to the Police Dog. We will invoice you via PayPal for the fee. You will be required to provide candy and decorations. We will email you prior to the event with more details about when set-up is and what time to be there! We look forward to seeing you!
